Since the end of 2016, South Eastern Hampshire CCG has been working with 20 health and care organisations across Hampshire and the Isle of Wight to determine how best to meet the many opportunities and challenges facing the local health and care system. This has focused on both the need to empower people to stay well and also on how we can provide safe, high quality, consistent and affordable health and care to everyone.

Hampshire and Isle of Wight Local Digital Roadmap
The Five Year Forward View made a commitment that by 2020, there would be “fully interoperable electronic health records so that patient’s records are paperless”. This was supported by a Government commitment in Personalised Health and Care 2020 that ‘all patient and care records will be digital, interoperable and real-time by 2020’.
The Local Digital Roadmaps (LDR) programme aims to shape national policy and strategic investment through a detailed understanding of local health and care systems’ plans to use digital technology to drive sustainability and transformation. LDRs, aligned to Sustainability and Transformation Plans (STP), will provide the evidence base and justification to underpin new investment and increased optimisation of existing assets.
